Can I still watch the reel my friend sent me?

Yes. Opened from a DM, another app or a pasted link, that single item plays for 5 minutes. Swiping to the next one is blocked, and you get at most one new pass every 10 minutes.

What stops me from just turning it off?

A breathing timer you have to stay present for. Switch tabs or apps and it restarts. Then you type a sentence like "I am choosing distraction over my own goals right now", word for word, no pasting. If the sentence doesn't feel true, you'll close the tab instead. That's the design.
